Milkshake

High FODMAP

Serving sizes

Milkshake changes rating with portion size. Its rating is estimated from what it is made of, so the servings are described rather than weighed.

  • Small serving Moderate
  • Moderate serving High
  • Large serving High

A milkshake is High FODMAP, since it's built from milk and ice cream blended into one rich, creamy glass.

  • The lactose from all that milk and ice cream adds up fast: a small serving sits at moderate, and it climbs to high by a moderate serving and stays high at a large one.
  • Milkshakes come in many flavors and add-ins, and while the milk base is the main driver, syrups or mix-ins can shift the picture too.

If you're craving one, blending a lactose-free milk with a lactose-free ice cream lets you keep the creamy treat while easing the load.

Why milkshake is rated high

A milkshake is high FODMAP because of Lactose. It is milk blended with ice cream, two of the richest Lactose sources in the kitchen, so the load builds quickly in one drink. The rating is estimated from what a milkshake is made of rather than from a measured serving, and the qualitative ladder shows the climb: a Small serving reads MODERATE, a Moderate serving reaches HIGH, and a Large serving stays HIGH. That Small serving is the only rung below high, and it is still moderate, not low.

What changes your risk

  • Serving size The ladder runs from MODERATE at a Small serving to HIGH at a Moderate serving and a Large serving, so the size of the shake decides how hard it lands.
  • Lactose-free processing Lactose-free milk is ordinary milk with the lactase enzyme added, which splits the Lactose. A shake built on lactose-free milk and lactose-free ice cream drops the Lactose problem, though it is still dairy and not suitable for a milk allergy.
  • Stacking Lactose adds up. A milkshake with a cheesy meal made with soft cheese, a creamy sauce, or a scoop of ordinary ice cream on the side stacks the same subgroup into one sitting.
  • Add-ins Honey, mango, malted mixes and some syrups bring their own FODMAPs, so a loaded shake can climb further than the dairy alone.
  • Extra cream Whipped cream or extra scoops of ice cream on top add more Lactose to a drink that is already high.

Swaps

The cleanest swap is a shake made with lactose-free milk and lactose-free yoghurt, blended with frozen firm just-yellow banana, strawberries or blueberries, and sweetened with maple syrup or table sugar. Peanut butter blends in well for a richer, nutty shake. Keep in mind that lactose-free products are still dairy and are not for anyone avoiding milk because of allergy. Ordinary milk, cream, ice cream and regular yoghurt are not swaps here, since they carry the same Lactose.

How to lower the load

At home, freeze the banana or berries ahead of time so the shake comes out thick without needing ice cream, then blend with lactose-free milk. Check flavour syrups and mix-ins for honey, high fructose corn syrup or fruit like mango. When ordering out, most shake bars make their shakes from ordinary milk and ice cream, so ask whether a lactose-free milk is available, and treat a standard shop shake as high.

Common questions

is a milkshake low fodmap

No. A milkshake is rated high FODMAP overall, estimated from what it is made of. A Small serving reads MODERATE, and it reaches HIGH by a Moderate serving.

is a milkshake high in lactose

Yes. Lactose from the milk and ice cream is the reason it rates high.

can i make a low fodmap milkshake with lactose free milk

Yes. Lactose-free milk and lactose-free yoghurt have the Lactose already split, so blending them with frozen firm banana or berries makes a shake that avoids the Lactose problem. They are still dairy, not dairy-free.

can i have a milkshake on the elimination phase

A regular milkshake is best left out, since even a Small serving reads MODERATE. A shake built on lactose-free milk is the way to keep the treat during elimination.

Milkshake is an estimated rating: it is inferred from the food's ingredients and composition and cross-checked, not measured directly in the papers below. The low, moderate and high cutoffs are still those of Varney et al. (2017).
