Iced Coffee

Moderate FODMAP

Serving sizes

Iced Coffee changes rating with portion size. Its rating is estimated from what it is made of, so the servings are described rather than weighed.

  • Small serving Low
  • Large serving Moderate

Iced coffee lands at Moderate FODMAP, since it's usually built on a generous pour of milk that adds up in a big cup.

  • The milk brings lactose, so a small serving stays low while a large, milky one climbs to moderate as that lactose builds up.
  • Iced coffee comes in all sizes and styles, and the more milk in the glass, the higher the load, so the base you choose is what really drives it.

Making it with lactose-free milk lets you enjoy a larger, cooling cup without the lactose stacking up.

Why iced coffee is rated moderate

Iced coffee is moderate FODMAP overall, and lactose from the milk is the reason. The coffee itself is not what moves the rating: it is how much ordinary milk is poured over the ice. The rating is estimated from what a typical iced coffee is made of rather than a measured serving, and the ladder shows where the milk starts to count: a "Small serving" reads low, while a "Large serving" climbs to moderate. Because cafe iced coffees are often large and very milky, moderate is the honest overall answer, and the size you order matters.

What changes your risk

  • How much milk The lactose comes from the milk, so the milkier and larger the drink, the higher it goes. A "Small serving" is low on the ladder, and a "Large serving" is moderate.
  • Lactose-free milk Lactose-free milk is ordinary dairy with the lactase enzyme added, which splits the lactose before you drink it. It is still dairy, so it is not suitable for a milk allergy or a dairy-free diet, but it takes the lactose out of the picture.
  • Cream, ice cream and condensed milk Iced coffees topped with cream, blended with ice cream or sweetened with condensed milk add far more lactose than plain milk would, so these are the versions to be most careful with.
  • Sweeteners and syrups Honey, agave and sugar-free syrups made with sugar alcohols bring their own FODMAP load into the cup. Table sugar or maple syrup does not.
  • Stacking An iced coffee with a breakfast of ordinary yoghurt, soft cheese or a milky cereal piles lactose into one sitting, even when each part looks modest on its own.

Swaps

The cleanest swap is the same drink made with lactose-free milk, which keeps the creamy iced latte texture while splitting the lactose. Black iced coffee or cold brew with no milk sidesteps lactose entirely. Sweeten with table sugar, maple syrup or rice malt syrup rather than honey, agave or sugar-free syrups. Do not treat an iced coffee made with cream, ice cream or condensed milk as the lighter option, because those carry more lactose than milk does.

How to lower the load

When ordering, the size and the milk are the two choices that matter: ask for lactose-free milk, and keep the drink to a Small serving rather than assuming the menu sizes match the ladder. At home, brew coffee ahead, chill it, and add lactose-free milk over ice, so you control both the milk and the sweetener. Cold brew concentrate keeps in the fridge for several days and lets you build a drink in moments. Check bottled ready-to-drink iced coffees for milk solids, cream, condensed milk and sweetener on the label.

Common questions

is iced coffee low fodmap

Iced coffee is rated moderate FODMAP overall because of lactose in the milk. A "Small serving" reads low, but a "Large serving" climbs to moderate, so the size and the milk decide where it lands.

is iced coffee high in lactose

Lactose is the subgroup behind its rating, and it comes from the milk rather than the coffee. Cream, ice cream or condensed milk add even more.

can i make iced coffee low fodmap with lactose-free milk

Lactose-free milk has the lactose already split by the lactase enzyme, so it takes away the reason iced coffee rates moderate. It is still dairy, so it is not suitable for a milk allergy.

what iced coffee can i order on low fodmap

A black iced coffee or cold brew with no milk, or any size made with lactose-free milk, avoids the lactose. With ordinary milk, the "Small serving" is the low rung on the ladder.

Iced Coffee is an estimated rating: it is inferred from the food's ingredients and composition and cross-checked, not measured directly in the papers below. The low, moderate and high cutoffs are still those of Varney et al. (2017).
