Fructose

High FODMAP

Serving sizes

Fructose changes rating with portion size. Its rating is estimated from what it is made of, so the servings are described rather than weighed.

  • Small serving Moderate
  • Moderate serving High
  • Large serving High

Fructose is High FODMAP, a pure fruit sugar with none of the balancing glucose that would normally help it go down easily.

  • With no glucose alongside it, fructose is the textbook source of Excess Fructose: it sits at moderate even at a small serving and climbs to high by a moderate serving and beyond.
  • It turns up as a sweetener in drinks, sauces, and baked goods, so scanning ingredient lists for it helps you spot where the load is coming from.

When you need a touch of sweetness, plain table sugar or maple syrup carries its fructose balanced with glucose, which sits much easier.

Why fructose is rated high

Fructose is rated high FODMAP overall, and the reason is about as direct as it gets: it is the sugar behind Excess Fructose. Sold on its own, with no glucose alongside it, all of it counts toward that subgroup. This rating is estimated from what it is made of rather than from a measured serving. The ladder never comes in low. A Small serving already reads moderate, and by a Moderate serving it is high, where it stays at a Large serving. No rung turns fructose into a low FODMAP ingredient.

What changes your risk

  • Serving size The ladder reads moderate at a Small serving, then high at a Moderate serving and a Large serving. Less means less load, but no rung reaches low, so cutting the amount is not a way to make it low FODMAP.
  • Hidden fructose on labels Fructose, crystalline fructose, fruit sugar, fructose syrup and high fructose corn syrup all turn up in sports drinks, flavoured waters, sauces, jams, cereal bars and baked goods. Each one adds to the same Excess Fructose load.
  • Stacking with fruit and sweeteners Honey, agave syrup, apple, pear, mango and watermelon also carry Excess Fructose. A fructose-sweetened drink next to any of them in the same sitting piles the load up fast.
  • Spreading it out When a product with fructose is part of the day, keeping other Excess Fructose foods out of that meal and spacing things across the day keeps any one sitting lighter.
  • Cooking and baking Heat does not break fructose down. A cake, glaze or sauce sweetened with it carries the same Excess Fructose after it comes out of the oven.

Swaps

When a recipe or drink calls for fructose, plain table sugar, maple syrup and rice malt syrup fill the same sweetening role without being built around Excess Fructose. Table sugar and maple syrup work in baking, dressings and hot drinks, and rice malt syrup suits sticky glazes and no-bake bars. Honey, agave syrup and high fructose corn syrup are not swaps: they are high in the same subgroup and only move the problem around. Sweetening with apple or pear purée has the same issue.

How to lower the load

Read the ingredient list rather than the front of the pack, because fructose often sits in products sold as low sugar or naturally sweetened, including flavoured waters, sports drinks, reduced-sugar jams and some snack bars. When baking at home, replace fructose with table sugar or maple syrup and keep the rest of the bake low FODMAP too, with gluten free flour or oats as the base. If a fructose-sweetened product does end up in a meal, keep the fruit on the plate to options such as strawberries, blueberries, oranges or kiwi, and leave honey, apple and mango out of that sitting.

Common questions

is fructose low fodmap

No. Fructose is rated high FODMAP overall, estimated from what it is made of, because it is the sugar that drives Excess Fructose. A Small serving reads moderate and a Moderate serving is already high.

how much fructose can i have on low fodmap

The ladder has no low rung: a Small serving is moderate, and a Moderate serving and a Large serving are both high. Tolerance is individual, and exploring it belongs with a dietitian during reintroduction.

what can i use instead of fructose in baking

Table sugar, maple syrup or rice malt syrup fill the same role. Honey, agave syrup and high fructose corn syrup are high in Excess Fructose too, so they are not swaps.

does baking with fructose make it lower fodmap

No. Heat does not break fructose down, so a bake or sauce sweetened with it carries the same Excess Fructose load after cooking.

Fructose is an estimated rating: it is inferred from the food's ingredients and composition and cross-checked, not measured directly in the papers below. The low, moderate and high cutoffs are still those of Varney et al. (2017).
